Job Description: Social Health Information Exchange (SHIE) Program Manager

Position Overview:
The SHIE Program Manager will lead and coordinate efforts related to the implementation and sustainability of the Colorado Social Health Information Exchange (CoSHIE) initiative in Northwest Colorado. This position bridges technology and human services by advancing equitable, secure, and standardized data exchange among healthcare, behavioral health, and social service providers. The ideal candidate will possess strong health IT knowledge, regional coalition-building skills, and a deep commitment to whole-person care and health equity.

Supervision Received:
The SHIE Program Manager is supervised by the Community Impact Director.
Supervision Exercised:
The SHIE Program Manager will be responsible for building and managing the THP SHIE team.
Key Responsibilities and Accountabilities:
Health Information Exchange Implementation & Management
- Oversee the deployment and integration of SHIE technologies including FHIR, HL7, OAuth2, and other interoperability standards.
- Manage the development and use of tools such as the Community Resource Inventory (CRI) and Community Resource Referral Platform (CRRP).
- Implement secure consent management, identity resolution protocols, and data-sharing workflows in compliance with HIPAA and state laws.
- Coordinate with technical teams to ensure system performance, security, and interoperability across platforms.
Regional Partnership & Stakeholder Engagement
- Build and maintain partnerships with clinical providers, CBOs, payers, and government agencies to advance SHIE goals.
- Represent the region in CoSHIE state-level forums and implementation workgroups.
- Facilitate user groups, advisory boards, and regional stakeholder work sessions to drive alignment and local engagement.
- Collaborate with diverse stakeholders to ensure community voice and equitable outcomes in system design and data use.
Strategic Planning & Policy Alignment
- Translate state and federal policy initiatives (e.g., HHS Call to Action on Health-Related Social Needs) into localized strategies.
- Monitor regulatory and funding landscapes to ensure alignment with compliance, privacy, and health equity mandates.
- Support development of regional roadmaps and performance dashboards that reflect CoSHIEβs vision and metrics.
- Align program implementation with broader organizational and community health equity strategies.
Project Management & Operational Oversight
- Lead multi-phase technical implementation efforts, including staff training, team user adoption, and sustainability planning.
- Develop scopes of work, budgets, and timelines in coordination with leadership and funders.
- Ensure timely and accurate reporting to meet federal and state grant requirements.
- Monitor and report key performance indicators (KPIs), ensuring continuous improvement and system effectiveness.
Capacity Building & Change Management
- Facilitate training and support for users of SHIE tools, including onboarding new partners and providers.
- Develop in-house IT capacity to reduce external vendor dependency.
- Promote a culture of continuous learning and innovation related to digital health and social care integration.
- Lead change management processes to ensure smooth adoption and long-term utilization of SHIE tools.

Qualifications:
Education & Certifications:
- Bachelorβs or Masterβs degree in Health Informatics, Public Health, Health Administration, or a related field.
- Preferred certifications: CPHIMS, RHIA, PMP, or other relevant health IT/project management credentials.
Experience:
- Minimum 5 years in health information exchange, health IT implementation, or related project management roles.
- Demonstrated experience in interoperability standards (e.g., FHIR, HL7), EHR integration, and social determinants of health data.
- Proven leadership of multi-sector initiatives and comfort navigating regional and state-level partnerships.
Skills:
- Technical acumen with secure data exchange platforms, APIs, and privacy frameworks.
- Strong facilitation, communication, and stakeholder engagement skills.
- Commitment to equitable care delivery and cultural humility in diverse settings.
- Analytical thinking and the ability to manage complex systems and data initiatives.

Salary Range: $60,000-$70,000 annually
